Respirator Fit test are required annually for any pesticide applicator using a pesticide that requires a respirator. This event is appointment based and you MUST pre-register to have a spot. Please call the office at 583-5161 or email Brandon Pike at bepike2@ncsu.edu to schedule an appointment.

Once you have an appointment you will need to fill out this Respirator_Medical_Clearance_Questionnaire if you need a paper copy please stop by the office to pick one up.

You must also bring your respirator with you to the fit test.

You must be cleanly shaven prior to coming to the fit test.

This Respirator Fit test is conducted by the NC Agromedicine Institute and they do charge a fee for this service so please bring a form of payment.
